Here you will learn how to manage the Practice Legal Info, Providers List, and Document Header. Who can edit/publish consents?
To review/edit/publish consents, log onto dashboard.touchmd.com, select “Legal”, and choose the desired consent. Note: to add and manage consents to TouchMD, the user will need the “Legal” access role. Practice Legal Info:
What is the Practice Legal Info?
The Practice Legal Info must be completed before consents may be added and managed. This will be the first step to complete when you open the Legal tab. The Practice Legal Info will display at the end of each consent within an e-signature disclaimer. This will notify patients that if they have any questions and/or concerns regarding the consent, that they should reach out to your practice directly.
E-Signature Disclaimer
Below is an example of the text that will be listed in the e-signature disclaimer portion. This Text will include your practice information rather than the example practice listed. Please note, this text may not be removed and/or altered. This text must be made available to patients to ensure their understanding of e-signature laws and because TouchMD is compliant with all e-signature laws
You have the option to sign this Agreement electronically or sign a paper copy of this Agreement. By signing electronically using any device, means or action, you consent to the legally binding terms and conditions of this Agreement. You further agree that your signature on this Agreement (hereafter referred to as your ‘E-Signature’) is as valid as if you signed this Agreement in writing. You also agree that no certification authority or other third party verification is necessary to validate your E-Signature, and that the lack of such certification or third party verification will not in any way affect the enforceability of your E-Signature or any resulting agreement between you and Avezen, LLC. You are also confirming that you are authorized to enter into this Agreement in your behalf. You understand that by selecting the ‘Decline’ button you have the option to have this Agreement made available to you in paper form for hand signing.
You acknowledge that you have access to an account with an internet service provider, and you are able to view or download a copy of the this Agreement by accessing your secure TouchMD account at https://patient.touchmd.com/ (requires the latest web browser), or by using the myTouchMD app (requires the latest iOS or Android version). Providers List:
What is a Providers List?
Utilization of this feature allows a user to create a list of providers that may be added as a select-from-list option to a consent. Essentially, it allows the individual assigning the consent to the patient to select from the list of providers who will performing the service/procedure or any other instance that a provider list may be needed in a consent. Usage of this feature is optional.
What may I also manage?
Note that you may arrange the order of the providers and/or delete providers. If you make any adjustments, be sure to select “Save Changes”

Document Header:
Where will the Document Header show?
The Document Header will only appear at the top of a completed consent (PDF).
Pro Tip:
Use a photo editing software (i.e., Photoshop, Paint, etc…) to size the image to the required sizing BEFORE uploading. (Begin with a high res version for best results)
What is padding?
Padding is the space placed around your logo to help center the logo on the completed consent. As you add your logo you may need to add padding to help your logo look the best on completed consents.
Consent Signature Dates:
When signing a consent it is possible to have the date and time automatically listed underneath the signature on the completed consent (PDF). You may opt to just have the Reviewer Signature with a date and time, to have just a regular signature with a date and time, to have both signatures with a date and time, or to not have a date and time listed for either type of signature.
